Message from Dr. Smith

Dear Patients, 

It is with mixed emotions that I announce my retirement from 27 years of private practice as of May 31, 2025.   I have had the honor and privilege of caring for your health for many years, and the decision to pursue other opportunities has been extremely difficult. Your trust and loyalty have meant more than can easily be expressed, and the relationships built with you will be greatly missed. 

I know that this decision will come as a surprise to many of you, and I apologize for the inconvenience it will cause. 

I encourage you to establish a relationship with a new physician in the coming months to assume your care after I have left Mountain View Family Physicians. 

At this time the current physicians at our office have essentially full practices, however they are happy to announce that there will be 2-3 new physicians joining the group. There will be limited availability of these physicians to assume your care.  Please refer to our website for updates on the new physicians. 

If you would like to continue your care with Mountain View Family Physicians please inform our office by the end of May so that the staff can do their best to accommodate your wishes.  

I understand that some of my patients will find it easier to relocate their care to a physician closer to their home or work.  Please have your new physician’s office reach out to my medical records department so we can transfer your medical records to them.  

There are many physicians accepting new patients in the primary care offices available through Honor Health [623-580-5800], Banner Health [833-252-5535], and Dignity Health [855-697-0781].  Office locations throughout the Phoenix area can be seen on their websites.
