Message from Dr. Lim

To all my patients,

It is with mixed emotions that I share with you my decision to leave the family practice that I have had the privilege of being a part of for the past eight years. This has been a difficult decision, as it means saying goodbye to the many wonderful patients and relationships I’ve had the honor of serving.

I understand this news may come as a surprise, and I sincerely apologize for any inconvenience this may cause. Your trust and support over the years have meant the world to me, and I will genuinely miss being part of your healthcare journey.

As I transition out of my role, I encourage you to begin exploring other healthcare providers who can continue to care for you moving forward. If you would like to continue your care with our practice, please let the office know by the end of May so we can help make this transition as smooth as possible, as May will be my last month at Mountain View Family. However, please note that availability will be limited as many of the remaining physicians are at full capacity. For updates on new providers joining the team, please check our website.

I understand that some of you may prefer to find a physician closer to home or work, and I fully support that decision. If you choose to transfer your care, please have your new provider contact our medical records department to ensure a seamless transfer of your health information.

For those seeking a new physician, there are many excellent providers accepting new patients within the networks of Honor Health, Banner Health, and Dignity Health. You can view available locations on their websites.

I am truly grateful for the opportunity to have been your physician. I deeply appreciate your understanding and patience during this transition and wish you all the best in your continued health and wellness.
